Final Report / Progress
Report Guidelines
Your
report should include the following information:
A financial report documenting that the grant
funds were used for the purpose intended, and a copy of
the final program budget with actuals.
A program progress report briefly describing
the activities carried out under the terms of the grant,
as well as any changes that occurred that differed from
the original proposal.
A summary of outcomes that were achieved,
in terms of real change in the lives of your constituents.
